Experts report one of two remaining types of polio virus may be eliminated in Pakistan

ScienceDaily (Nov. 13, 2012) ? Polio cases worldwide reached historic lows in 2012, and for the first time there were no new outbreaks beyond countries already harboring the disease, leaving researchers confident that a massive and re-energized international campaign to eradicate polio is on a path to success, according to presentations November 13 at the annual meeting of the American Society of Tropical Medicine and Hygiene (ASTMH).

Globally there were 177 polio cases through October 2012, a drop from 502 during the same period last year.

Despite the dramatic drop, polio experts noted challenges in Pakistan posed by parents who refuse to vaccinate their children and in Nigeria where polio cases more than doubled in 2012 and threatened to re-infect currently polio-free countries. Pakistan, Nigeria and Afghanistan are the only countries where polio remains endemic and are the battle grounds of efforts to make polio only the second human disease, after smallpox, to be completely eliminated.

Steven Wassilak, MD, a medical epidemiologist and polio expert at the US Centers for Disease Control and Prevention (CDC), said new data from Pakistan show that of the two types of wild polio virus (WPV1 and WPV3) circulating in the country, the one known as WPV3 -- or Type 3 -- is close to being eliminated.

"There have not been any Type 3 cases reported for six months, which is the longest gap in incidence there to date," he said. "CDC works with Pakistan officials to monitor different chains of transmission over time and Type 3 is now down to only one chain, which is an indication that we are close to breaking the last link of Type 3."

CDC, which has led the effort to establish a global network of laboratories to track and sequence the genome of the wild polio virus, is one of four partners spearheading the Global Polio Eradication Initiative along with Rotary International, UNICEF and the World Health Organization (WHO), with great support from the Bill and Melinda Gates Foundation. In September, the partners along with leaders from Pakistan, Nigeria, and Afghanistan met at United Nations Headquarters in New York to reaffirm the Emergency Action Plan against polio, launched in May. The initiative includes a "surge of human resources" involving 4,000 people who have been deployed to complete the eradication effort.

New Study Finds Vaccine Refusals Remain Barrier to Elimination

At the ASTMH conference, Anita Zaidi, MD, a pediatrician at Aga Khan University in Karachi who serves on Pakistan's National Immunization Technical Advisory Group, is presenting new data showing that while Pakistan has made major progress against polio by expanding immunization campaigns, the remaining challenge is not one easily solved by additional resources.

"We found that in Karachi, a key reason children fail to get immunizations is not due to lack of access, but because their parents refuse to participate," Zaidi said. "That is a big challenge and not something that can be overcome only by expanding immunization campaigns."

In a study published in the Bulletin of the World Health Organization (WHO), Zaidi and her colleagues found that in Karachi, "parent refusal was the most common reason given for the failure of children to participate in two recent polio supplementary immunization activities," accounting for 74 percent of missed immunizations.

While opposition to polio vaccinations has been widely reported in the remote, restive tribal territories in the north, less examined, Zaidi said, has been the situation in the urban environs of Karachi, which her study describes as "the only megacity in the world that has not succeeded in interrupting polio transmission." Nationwide, Pakistani health officials believe vaccine refusals, driven by false rumors that immunizations cause sterility and are contaminated with HIV, actually are declining. But Zaidi said that recent events in Karachi suggest the intensity of those who remain opposed appears to be rising.

She pointed out that in just the last few months, a health worker in Karachi involved in the polio campaign was killed and the child of another was kidnapped (though later returned unharmed). Zaidi said that she recently participated in a polio seminar in Karachi where concerns about violence prompted the organizers to avoid any publicity. Given the current volatile environment and continued evidence of widespread viral transmission in sewage sampling, Zaidi said that while Pakistan is making considerable progress in reducing polio cases, eradication by 2013 appears unrealistic.

"Efforts should focus on building trust through grassroots efforts using community elders in populations with high vaccine refusal rates," she said. By providing vaccination at mass transit sites such as bus routes used to travel up-country throughout the year, we can at least isolate the viral reservoirs and make sure we avoid what happened last year, which was exporting the virus to China."

In Nigeria, Working With Traditional Leaders, Nomadic Groups

Meanwhile, opposition to immunizations is also a major challenge in Nigeria, where the number of polio cases has risen for the second straight year, said Adamu Nuhu, MD, with Nigeria's National Primary Healthcare Development Agency. He said cases are at least confined to the northern part of the country where opposition to immunizations is rooted in religious or political differences and, as in Pakistan, has been stirred by rumors of vaccine-induced sterilization and HIV infection.

"We are working now with traditional leaders in the north who are respected by local people to change the perceptions of polio immunization and encourage more participation in immunization efforts," Nuhu said. "There is evidence that overall, immunization rates among children at risk are rising to 80 percent."

He described a campaign that is now literally going house-to-house to identify family decision makers and talk with them about the importance of polio immunization. Also, health officials -- who are working closely with international partners including the WHO, UNICEF and the CDC -- are pursuing new strategies to improve immunization coverage in nomadic groups.

"Even though they are often moving, they have a leadership structure and we can work through these leaders to provide polio immunizations," Nuhu said. "But we understand that you will not be able to immunize any children unless you can reach them on their pastoral routes and camps."

The persistence of polio in Nigeria -- in 2012 the country has documented 99 cases -- is especially worrisome because cases in Nigeria have spread in the past to Sudan, Chad, and 23 other countries. All these countries are now once again polio-free except for Chad. The CDC reported in October that Chad could interrupt wild polio transmission by the end of this year, but that failure to stop transmission in Nigeria could prompt new outbreaks.

Vaccine Refusal: Lessons from India?

The CDC's Wassilak said the refusal by parents in Pakistan and Nigeria to vaccinate their children presents a challenge to eradication. "Karachi is a densely populated area where you need a high degree of immunity to interrupt polio transmission, so every child counts," he said.

But Wassilak said vaccine refusal is not an insurmountable barrier to global eradication.

"We saw similar problems in India, and while they were not resolved overnight, eventually we saw immunization coverage increase and polio cases halted," he said. "It requires working more closely with community leaders and greater political commitment at all levels, which is what we are seeing in both Pakistan and Nigeria."

India has not reported a polio infection since January 2011 and the entire Southeast Asia Region of WHO could be certified polio-free in 2014 if no new cases arise. Wassilak said advisors from India are now working in Nigeria to share their lessons learned. Meanwhile, the CDC is providing technical support to the global eradication effort, tracking the different types of polio that are circulating in the affected countries and training volunteers to assist in polio vaccination campaigns.

"This urgent international push for polio eradication means that soon no child will be hurt by this disease. The relentless drive to alleviate pain and suffering is the spirit of this Society and permeates every session and every hallway conversation at this meeting," said ASTMH President James W. Kazura, MD, FASTMH. "We will get this done."

Green Party candidates arrested at debate

(AP) ? Police say the Green Party candidates for president and vice president have been arrested at the second presidential debate.

Nassau County, N.Y. police say Jill Stein, the Green presidential nominee, and Cheri Honkala, the vice-presidential nominee, were charged with disorderly conduct as they tried to enter the debate site at Hofstra University.

The Green Party says in a statement that Stein and Honkala were walking with supporters toward the Hofstra campus Tuesday afternoon when they were met by uniformed police officers. Stein and Honkala then held an impromptu press conference in which Stein called the debate a "mockery of democracy."

A third person was later charged with disorderly conduct.

Manatee's toughest jobs: Behind the scenes with the bedbug exterminator

Imagine that it's as hot as a sauna in your house.

Around 140 degrees -- hot enough to melt candles and cosmetics, like the inside of a car with the windows rolled up on a sunny day.

Now imagine rearranging all the furniture in your house in that heat. For good measure, reorganize everything inside your closets and cabinets, too. Once you're finished, do it all over again.

Sound crazy? That's what the bedbug crew at Keller's Pest Control in Bradenton do nearly every day. But they're not redecorating, they're eradicating. Bedbugs.

They're a largely unseen menace, lurking in the dark crevices of places where

people normally feel the safest. Once the blinds are drawn, the lights switched off, and we're snug in our sheets or easy chairs, basking in the glow of the television, the bedbug's blood feast begins.

They can be no thicker than a credit card and as small as the tip of a blunt pencil. For those who have been unlucky enough to harbor them -- unwittingly inviting them into their home, carried on clothing, bedding, luggage or furniture -- it can become nothing short of a nightmare.

Rodney O'Quinn, owner of Keller's Pest Control, has seen it first hand. People often call in and break down on the telephone, crying as they describe how they can't sleep at night, or tell of children covered in welts -- allergic reactions caused by the tiny blood-eating insect's bites.

O'Quinn's company has treated a diverse array of local homes and edifices, from hotels and condos to offices and nursing homes. They can be found pretty much anywhere people congregate, and getting rid of them can be expensive and exhausting.

About four years ago, after trying conventional treatments for years with varying degrees of success, O'Quinn invested in a thermal-eradication solution. So far it has been a win-win scenario for both O'Quinn and his customers.

As bed bug infestations continue to rise across thecountry, so does the volume of calls Keller gets from people throughout the Tampa Bay area, including right here in Manatee County.

Keller's Pest Control is unique in that they are the only company within four neighboring counties to provide this method of bedbug eradication. Their work takes them from Charlotte County up to Tampa and east to Orlando.

As O'Quinn explains, all life phases of the insect will perish when exposed to high temperatures for certain lengths of time. But once the temperature hits 126 degrees, it's more or less instant death for a crawling bedbug.

Crank up the temperature to about 140 degrees and heat every surface of every object or structural element inside a living space for at least a few hours and it's a guaranteed no-win scenario for the bugs and their offspring and eggs.

It is meticulous work carried out under adverse conditions. They have to be sure to distribute the heat thoroughly and consistently. That means moving everything -- couches, mattresses, bureaus, china cabinets, closets stuffed with clothing, sheets and bric-a-brac -- all getting shifted and rotated around in those scorching temperatures to ensure heat penetration and bug death. To keep safe from overheating and overexertion, the bedbug crew always works together and try to limit their exposure to the heat for 10 minutes at a time.

Equipment setup takes about an hour and a half, unless there's stairs. For each service call, The Keller's Pest Control crew will route hundreds of pounds worth of pressure-resistant hoses and connect them to portable radiators strategically placed throughout the home or building. The hoses circulate heated fluid pumped from a trailer-mounted furnace.

The radiators crank out heat at temperatures exceeding 140 degrees and work in conjunction with a dozen industrial fans positioned to help create a convection current throughout the rooms being treated.

Remote temperature sensors are placed inside each room and in critical spots, such as bed frames or underneath furniture, and are monitored and recorded by a computer that allows the workers to visualize any problem cold spots and redirect the flow of heat. In one visit they can treat up to 3,000 square feet.

In situations where adjoining apartments or condos may be the source of a bedbug infestation, additional prevention and treatment steps are taken to eradicate the pests, including the use of bedbug sniffing dogs trained to detect the insect's tell-tale sweet and musty odor, which is oftentimes compared to rotting raspberries.

As O'Quinn and his crew work their bedbug mojo, they consume liters of water and sweat through every layer of clothing. Then, hours after they begin, they have to reverse the process -- unhooking and removing all the heavy equipment after allowing it to cool to below scorching temperatures.

Once that's finished, everything inside the home back goes back to its proper place. No easy task itself, as all the contents of the home are splayed about in various configurations. For a mind addled by extreme heat and exertion, it seems like a wicked game of memory: On top of it all, one has to remember where everything goes.

So next time you remark on how hot Florida can get, just think of these guys. Every time they walk outside from their lovingly crafted, honest-to-goodness bedbug killing inferno to take a break, for them, the sultry 90-degree weather feels quite refreshing.

But despite the grueling details of the job, for O'Quinn and his employees, there is satisfaction to be found in a job well done. Bedbugs can wreak havoc on both a physical and psychological level, and getting rid of them goes a long way in helping to return someone's life to normal -- where normal means being able to have a good night's sleep without being tormented by the hunger-driven visitations of the lowly blood-sucking bedbug.

It doesn't hurt that it pays well, either.

Construction Industry Improving, Challenges Still Ahead

Activity improves, but industry faces slow job growth and higher prices

MINNEAPOLIS?(BUSINESS WIRE)? Despite business improvements in the overall construction industry, contractors and equipment distributors expect the overall downturn in the construction industry to last at least until late 2013, according to the latest Construction Executive Survey from Wells Fargo Equipment Finance, part of Wells Fargo & Company (NYS:?WFC) . Conducted in August, the quarterly survey recorded the responses of approximately 250 construction industry executives across the U.S.

?The survey results demonstrate some improvements in the construction industry over the past 12 months, but construction executives are clearly still cautious,? said John Crum, senior vice president and national sales manager of the Construction Group at Wells Fargo Equipment Finance, Inc. ?Although the industry is still facing challenges, we expect to see modest improvements in the non-residential sector. At Wells Fargo, we are encouraged to see this year-over-year increase in activity and remain committed to serving our customers? construction equipment financing and leasing needs.?

Highlights of the Q3-2012 Construction Executive Survey:

  • Construction activity improved:?Nearly half of construction executives (47.8%) said construction activity was ?somewhat higher? or ?much higher? than a year ago. In the 2011 survey, 38.4% said activity was higher and in 2010, only 28.5% said activity had improved over the previous year.
  • But full recovery could take a while:?Although executives acknowledge year-over-year improvement in construction activity, a full 61.0% said the downturn in the U.S. construction industry will not be over for at least another year. In the 2011 survey, that number was 82.2%.
  • Job growth is tentative: 43.6% of the construction executives said their company is not hiring new employees within the next six months. While 41.0% said their company would hire some workers in the short term, 15.5% said they may or will have to reduce their workforce.
  • Prices are rising:?Four out of five respondents (80.5%) said they are seeing ?somewhat higher? or ?much higher? equipment prices compared to a year ago. More than 75% (76.2%) said that prices for materials are ?somewhat higher? or ?much higher? than a year ago.

The Q3-2012 Construction Executive Survey continues the longstanding practice by Wells Fargo Equipment Finance, Inc. and its predecessors to publish primary research findings for the construction industry. You Can Lose Weight With This Weight Loss Advice : Premier Diet ...

Posted by haleyjonas on Friday, October 5, 2012 ? Leave a Comment?

Getting rid of excess weight can be a frustrating task. Weight loss is often mystifying; you do well for awhile and feel encouraged, then level off and wonder what is wrong. Sometimes, a plateau is a sign that you have to do something different. Here we?ll discuss a few things that you can try to work past your weight loss plateau and continue to lose weight.

An excellent way to shed some pounds is to go hiking in the great outdoors. In addition to enjoying the great outdoors, you will burn a lot of calories. The more strenuous and difficult the hike is, the greater number of calories you will burn.

Weight loss doesn?t stop after exercise; focus on losing weight while you are at work or at a party. Begin by selecting fruits and vegetables over foods that contain more calories. Doing this will allow you to take part in the festivities while sticking with your diet plan. Avoid making a big deal about the diet you are on while you are partying; just find ways around it.

TIP! An easy way to help lose weight is to reduce the size of the plate you use. You?re more likely to accidentally overeat if you use large dishes.

Staying active is a great way to successfully lose weight. Sit down as little as possible during the day. This keeps you metabolism high for the whole day, and makes it so that you can eat more calories per day and continue to lose weight.

Do not skip any meals. Try to eat about three balanced meals each day. You can eat snacks in between but make sure to get the full array of nutrients in your meals. This will help your body maintain its rhythm.

Making enticing dishes of fruits and veggies will get you in the habit of eating them often, so have them with healthy sauces and dips. Fruits can be used as desserts or made into chutneys as a healthy accompaniment to low-fat meats or vegetables. Cutting down on processed sugars is one great way to eat healthier and lose weight.

TIP! Avoid the traditional, three meals approach. Large meals are harder to process and can increase your caloric intake during the day.

Try to only consume water following your breakfast and up until lunchtime. Water is the healthiest thing to drink. It doesn?t contain calories, fat, sugar or anything else you?re trying to get rid of through your diet and exercise plan. You can have a glass of juice or milk with breakfast, but try to restrict yourself to water for the rest of the day to help the pounds melt away.

It is common knowledge that drinking plenty of water can increase your weight loss. If you want to really speed up your metabolism, then you should drink ice-water. Cold water makes your body work to raise it?s core temperature which in turn elevates your metabolism rate.

One out-of-the-box weight-loss idea is to dress and decorate with an emphasis on blue. Studies have shown that this color helps reduce the urge to eat. Try placing a blue vase on the table where you eat most of your meals to see if it works. Warmer tones, like orange, red or yellow, should be avoided since they encourage you to eat. Visual cues affect our eating habits more than people probably realize. Keep this valuable information in mind when you are choosing plates to use at a meal or even when you are figuring out what color to wear.

TIP! A good way to stay motivated is to find a friend to pair up with during your weight loss journey. Both of you can use each other as motivation, especially when one becomes tired or lazy while also offering great support when it comes to eating healthy.

Lose Weight

Use lower calorie versions of your favorite foods in order to help you lose weight. Hunger and cravings for specific foods cause many people to abandon their diet plans. You could choose the reduced-calorie types of your favored foods, so you still get to enjoy them without feeling deprived and lose weight in the process.

Stress can sabotage your diet. There is a great deal of temptation with regards to unhealthy food options whenever you are feeling stressed. Living a happier and stress-free life can help you focus on your goals and keep you on track.

TIP! Try taking a photo in your underwear, even if it is uncomfortable, so that you can see what your weight looks like. Looking at the ?before? picture can prevent you from backsliding if you don?t feel like working out.

One good way to help you stick to a diet is to drink at least eight glasses of water daily. Water is calorie free, and satisfying your thirst that way stops you from drinking something less diet friendly, like soda or juice. Sugary drinks contain a lot of calories and can hinder your weight loss.

When you first start your diet, try focusing on maximizing your nutrition in a 2000-calorie per day diet. Pay attention to your meals, and make certain that you get what you need, starting with Vitamin A and ending with Zinc. If you realize that these meals are depriving you of these vitamins and nutrients, you may want to think of switching up your diet or adding a multivitamin to your routine.

Get rid of any clothing that you wore before you lost weight. Keeping those clothes can sabotage your weight loss attempts. When you rid yourself of these clothes, you acknowledge that you will never be that heavy again.

TIP! If you live near a coast, running along a beach is a great way to exercise to lose weight. The sand is more resistant than the grass, making it a better thing to run on.

Losing weight is easier if you get cardiovascular exercise. Exercises that elevate the rate of your heart are considered to be cardio, and they include running, walking, and bicycling. Your body burns fat most effectively during periods in which your heart rate is elevated. Make it a point to engage in half an hour of cardio at least 3 times per week.

Lose Weight

Giving up sugar is the best way to lose weight. Cutting all foods that contain added sugar from your diet should be possible. Replace these foods with natural sources of sugar, like fruit or yams.

TIP! Foods that are low calorie versions are usually easy to get. Pick low fat ice cream or less cheese on that pizza on the next occasion you go for dinner.

If you are going to lose weight, try different things. You can indeed lose weight.

GoDaddy Web outage takes out small-business sites for a time | The ...

Tech ? Service restored after several hours as cause investigated.

Thousands and possibly millions of websites hosted by GoDaddy.com went down for several hours on Monday, causing trouble for the mainly small businesses that rely on the service.

A Twitter feed that claimed to be affiliated with the "Anonymous" hacker group said it was behind the outage, but this couldn?t be confirmed. Another Twitter account, known to be associated with Anonymous, suggested the first one was just taking advantage of an outage it had nothing to do with.

GoDaddy spokeswoman Elizabeth Driscoll said the outage began shortly after 1 p.m. EDT. By around 5:50 p.m. EDT, the GoDaddy.com website and sites hosted by the company were back up and running. Driscoll had said the company was investigating the cause.

GoDaddy.com hosts more than 5 million websites, mostly for small businesses. Websites that were complaining on Twitter about outages included MixForSale.com, which sells accessories with Japanese animation themes, and YouWatch.org, a video-sharing site.

Catherine Grison, an interior designer in San Francisco who operates the site YourFrenchAccent.com, said she had to stop sending emails with her website link in them while the outage was ongoing. The site is where she displays her portfolio of work.

"If I have no visuals I have nothing left except the accent," said Grison, a native of Paris. She said she was already shopping around for another site host because she was unhappy with GoDaddy?s customer service.

Earlier, Kenneth Borg, who works in a Long Beach, Calif., screen printing business, said fresnodogprints.com and two other sites were down. Their email addresses weren?t working either.

"We run our entire business through websites and emails," Borg said.

The business even takes orders from its two physical stores through the Web, so clerks had to use their personal email addresses to send in orders to the printing shop, causing an administrative headache, Borg said.

Champion Jose Aldo, ex-champ Rampage Jackson most recent victims of UFC injury bug

After fighters in the main and co-main events were forced Tuesday to withdraw from UFC 153 with injuries, only one thing remained certain: UFC president Dana White will, under no circumstances, postpone the card scheduled for Oct. 13 in Rio de Janeiro, Brazil.

Other than that, though, little is known about what will happen to the pay-per-view card after featherweight champion Jose Aldo (ankle) and ex-light heavyweight champion Quinton "Rampage" Jackson (elbow) were forced to pull out.

Aldo was originally scheduled to headline against Erik Koch at UFC 148 in Calgary on July 21, but had to pull out of that fight with an injury. Then, last week, Koch was injured and had to pull out of UFC 153. The UFC replaced him with former lightweight champion Frankie Edgar.

The Aldo-Edgar fight added a lot of sizzle to the top of the card, which also included a highly anticipated light heavyweight match between Jackson and the highly regarded Glover Teixeira.

First, White tweeted Tuesday that Jackson had to pull out because of an injury, reportedly to an elbow. Then, minutes later, he tweeted the news that injuries Aldo suffered when the motorcycle he was driving was hit by a car would force him to withdraw from the card.

A report surfaced later Tuesday on a Brazilian website quoting Aldo as saying he wasn't injured and would still fight.

I do not know anything about that.?I'm fine, I just train. No [the accident didn't hamper my training], I'm training normal. For me there was nothing spoken. Even on one leg I would fight.

A UFC spokeswoman returned a call for White. She said that Aldo had been told to keep quiet about his injury until the UFC could make a determination about what to do and that Aldo was unaware White released the news. She said he was just following orders when he denied being off the card.

[Related: Glover Teixeira without an opponent after 'Rampage' hurts elbow]

After having last month been forced to cancel UFC 151, which was scheduled for Sept. 1 in Las Vegas, because of an injury to Dan Henderson, White was determined to never cancel another show. That was the first cancelation in UFC history. Reports are that the cancelation cost the UFC more than $20 million.

White said Tuesday during a brief stop at the Yahoo! Sports offices in Santa Monica, Calif., that he had just learned of the news and didn't have anything to announce.

The only thing to be sure of is that he will under no circumstances cancel this card.

5 signs that PR pros are impacting business results ? Get In Front ...

Public relations is coming into its own and the industry appears more than ready to embrace a new level of respect.

How do I know this?

Consider the following?points:???

1. PR is more strategic than ever. There?s an increased understanding that a broader view of business is required in today?s marketplace. PR pros who fully grasp how their work directly impacts sales and marketing are sitting at the table with the C-suite. Messaging and money go hand-in-hand.?

2. PR pros are?communicating?like project managers. We?are tracking the?minutiae in?our assignments.?We are in constant communication with team members. Our work is completed on time and under budget.?That?s because we can course correct in the midst of a project instead of running out of time and resources.?We?re?using online spreadsheets, graphics, and images to track progress, next steps, and metrics.?Communicators are getting better at this thing called communication.

3. PR pros?the good ones, anyway?are buying into the?vision. These are the passionate ones;?the folks?who are curious and?courageous. Those who lack passion, including managers,?are crap magnets that are blind to the?company vision. Passionate PR pros understand the work that needs to get done every day to achieve more and compete in a fierce market. They surround themselves with like-minded people, leaving no room for the mediocre.???

4. PR pros are well-versed?in more than just one language.??The smart ones are able to speak to their clients in their own industry language. Whether it?s Software-as-a-Service, designer jewelry, healthcare, or aerospace engineering, PR stars shine brightest when they can hold their own and?engage?with niche clients.??These PR leaders?exude the confidence and knowledge to not only be invited to?sit at the?proverbial table, but to actually offer ideas and insights as a valued member of the team.?Courtesy seats with the C-suite rarely exist.??

5. PR pros recognize what is required?to service business-to-business clients.?Delving into the needs of the customer?s customer helps solidify B2B relationships.?Creativity and projects move to deeper levels and vertical markets, which has customers coming back, and offering referrals.?It?s simple.? You get it.????

Business and motivational speaker Denis Waitley said:??Your success depends on how well you think. You are not paid to collect, sort, store or retrieve information, although you do these things every day. You are paid to interpret that information and create and implement new ideas.?

As we head into the last quarter of 2012, it?s clear that PR means business.?Would you agree?

How breast cancer spreads

ScienceDaily (Sep. 10, 2012) ? The invasion of cancer cells into the lymph vessels that connect the breast to surrounding lymph nodes is the first step leading to the metastasis, or spread, of cancer throughout the body. Metastasis is the primary cause of breast cancer deaths. Surprisingly little is known about the control of this process and how it might be interrupted to prolong the lives of women with breast cancer.

In a study to be reported Sept. 10 in the Proceedings of the National Academy of Sciences Online Early Edition, researchers at Johns Hopkins describe their discovery of how a protein responsible for cell survival in low oxygen can trigger the spread of cancer cells into the lymphatic system in a mouse model of breast cancer.

The researchers knew that like all solid tumor cancers, breast cancer cells can grow so densely that they end up starved for oxygen. To survive, cancer cells trigger the growth of new blood vessels by activating a protein called hypoxia-inducible factor 1, or HIF-1. "We've known that increased levels of HIF-1 are associated with increased tumor vessels and with patient mortality," says Gregg Semenza, M.D., Ph.D., the C. Michael Armstrong Professor of Medicine, director of the vascular program at Hopkins' Institute for Cell Engineering and a member of the McKusick-Nathans Institute of Genetic Medicine. "Now we've found that HIF-1 activity is directly responsible for the spread of breast cancer to the lymph vessels."

Working in mice injected with human breast cancer cells, which when left undisturbed grow into tumors that spread from the breast to the lungs, Semenza's team previously found that interfering with HIF-1 in these mice reduced growth of the primary tumor and prevented metastasis through blood vessels to the lung. "So of course we wanted to see whether blocking HIF-1 could affect lymph node metastasis as well," he says.

In new experiments, they injected mice with human breast cancer cells that were genetically engineered to knock down HIF-1 protein levels and, after 24 days, examined the mouse lymph nodes to see if the human breast cancer cells had spread. They found that compared to mice whose HIF-1 levels were left undisturbed, lymph nodes with knocked-down HIF-1 contained 76 percent fewer human breast cancer cells, supporting the idea that HIF-1 is somehow involved in the spread of breast cancer to lymph nodes.

To better understand how HIF-1 triggers this to happen, Semenza's team then starved human breast cancer cells of oxygen to see which of the genes involved in the growth of lymphatic vessels might respond to HIF-1. They found that the platelet-derived growth factor B gene -- PDGF-B -- was five times more active when oxygen was lacking. A closer look at the DNA sequence around the PDGF-B gene showed regions of DNA known to be recognized and bound by the HIF-1 protein. They tested this in cells and found that, indeed, HIF-1 protein binds to the PDGF-B gene and turns it on.

The team then took a closer look at PDGF-B to find out how it works once the gene is turned on. They found that PDGF-B that is made by breast cancer cells is pumped out of the cell and stimulates the growth of lymph vessels.

Treating the mice with either digoxin, which blocks HIF-1 activity, or imatinib, a cancer drug, reduced tumor size by 78 percent and reduced lymph node metastasis by 94 percent, although the researchers emphasized that more work must be done to determine whether these drugs will be effective in treating breast cancer patients.

"We're very excited by these results, having shown for the first time that HIFs are directly involved in the lymphatic metastasis of breast cancer," says Semenza. "These results provide experimental support for breast cancer clinical trials that target HIF-1 or PDGF-B." The first study of digoxin in women with breast cancer at the Johns Hopkins Oncology Center will begin later this year.

This study was funded by grants from the American Cancer Society and the National Cancer Institute (U54-143868) and by funds from the Johns Hopkins Institute for Cell Engineering.

Pedestrians Severely Injured in Phoenix Car Crashes | Personal ...

As experienced pedestrian accident lawyers in Phoenix, it is unbelievable how many car accidents happen each day. Many car accidents happen when drivers of motor vehicles are distracted and do not see the pedestrian. Pedestrians who are hit by motor vehicles are likely to suffer very serious or life-threatening injuries. Typically, these victims are left in critical condition. This is always a difficult time for the injured person as well as the family members.

Phoenix Car Crash and Pedestrian Collision Statistics

Pedestrians are often severely injured in car accidents, and those pedestrians involved in Phoenix car crashes are no exception. More than 29,060 collisions happen in Phoenix each year, according to reports out of the Arizona Department of Transportation (ADOT). These collisions result in light to severe and even life-threatening injuries for more than 15,050 people, pedestrians included. Pedestrians involved in Arizona pedestrian collisions statewide number nearly 1,500 every year, and the number of those injured typically totals about 1,230. Nearly 1,300 of the total pedestrian collisions in the state happen in urban centers like Phoenix where cars drive faster and more accidents occur overall in a single year.

Why Involve a Phoenix Car Crash Attorney

Pedestrians who are hit many times are not in the crosswalk, and what people may not know is that if a pedestrian is hit and is not in the crosswalk, this does not mean that they have no claim. Under Arizona law, if the driver is at all at-fault, then the driver still owes that percentage of fault to the victim or their family.

The Husband and Wife Law Team would do whatever needed to be done to help a pedestrian struck by a car to achieve justice and fair results. Settlements achieve justice and also help pay for medical treatments, personal suffering, and the lengthy amount of time a victim is likely to have to take off from work for recovery. If it is a fatal injury, the Husband and Wife Law Team work with the family to achieve justice. We would do everything in our power to make certain that whatever settlement received is a fair one. We offer free consultations to all injury victims. Call the attorneys at the Breyer Law Offices, P.C. now.

Are Dental care X-Rays Really Necessary? - Health & Fitness Article ...

The well known media has recently directed a lot more attention to the problem of dental x-rays. A recent study has come to gentle which shows some link between dental x-rays as well as brain growths. Although there are some challenges that this particular study faces in the gathering of the company?s data, I believe that media attention which assists the consumer become interested and much more informed concerning the services that they receive is always a good thing. With this idea planned, I would like to deal with some of the concerns I have received from sufferers in my dentist.

Dental x-rays usually are not always had to perform a dental exam. In fact, dental x-rays must only be used when they are necessary for establishing a benchmark for the new affected individual and/or when there is absolutely essential and need for diagnosis as well as treatment of dental or oral disease. A lot of factors are usually evaluated to determine the need for the sort and variety of radiographs. Some of these components include: get older, oral health, risks, current signs or symptoms. Never must a one-size-fits-all strategy be used intended for the need for dental radiographs.

There are a few types of dental exams carried out in a dental office. These a few exams are usually: a comprehensive, preliminary, or brand new client examination, a limited as well as problem-focused exam, plus a periodic as well as established affected individual exam. The comprehensive oral evaluation is completed when a brand new patient wishes to become an existing member and there is a need to begin a benchmark or an oral health reference for the future. Given that a more required set of details are being gathered to create a time frame point, it is often at these appointments more and more dental x-rays are usually taken.

Each time a limited as well as problem-focused exam comes about, there are fewer dental x-rays essential except for the specific areas involved. During these evaluations, the x-rays are simply one of many different tests as well as pieces of details that are examined to determine the setting of therapy.

A schedule or routine dental examination typically comes about during a cleaning or dental prophylaxis appointment. Numerous issues are evaluated within this exam. I prefer to divide our periodic oral evaluations into three sections. I think of the oral examination as being a group of friends and the evaluation being separated into three distinct categories. This particular exam incorporates a visual analysis of soft tissues (lips, mouth, cheeks, nicotine gum tissue as well as gingiva, and gum charting), hard tissue (tooth, jaw, stoppage or bite, and then an evaluation of any radiographs which were taken. Every portion of the evaluation offers beneficial data that assists to aid us, the specialist, perform a a lot more thorough as well as comprehensive analysis of the individual?s oral health. Every portion of the examination has its own value; however, while all three parts are present, the most comprehensive analysis of the affected individual can be performed.

While clinicians, the objective is always to provide solutions and information which assists each individual become an active participant in maintaining their oral health. Dentistry radiographs or x-rays are simply one instrument used to help detect illness, infection, first cavity as well as gum disease. When radiographs are used, proper protecting aprons along with thyroid dog collars should always be accustomed to help reduce exposure to vulnerable and susceptible tissues. While patients be involved in their dental care, that they ask questions to assist them understand why certain recommendations are produced. When sufferers become more educated, their high quality of care improves along with the partnership involving patient as well as dentist develops stronger.

German minister: Don't let up on reforms

(AP) ? Germany's finance minister is insisting that countries must not use the European Central Bank's plan to buy unlimited amounts of government bonds as an excuse to let up on economic reforms and deficit-cutting.

The ECB announced its most ambitious plan yet to ease Europe's financial crisis on Thursday. The bond purchases would help lower borrowing costs for countries struggling to manage debts.

The head of Germany's central bank has objected, arguing that the ECB is moving too far toward financing government deficits ? which is prohibited by the European Union treaty. Senior government officials have signaled their acceptance, however, while making clear that the decision doesn't change the path struggling countries need to take.

"We will only overcome the euro's crisis of confidence if we do not let up on reforms," Finance Minister Wolfgang Schaeuble was quoted Saturday as telling the Bild am Sonntag newspaper.

"It would be a serious mistake if the ECB decision were misinterpreted in the sense that we could now let up on our efforts," he added. "The opposite is the case."

The ECB's pledge of support came with strings attached: countries that want the central bank to help with their borrowing costs must first ask the 17-nation eurozone's existing bailout funds to buy their bonds. The ECB also wants the International Monetary Fund to be involved.

Even so, the prospect of unlimited bond-buying, whatever the conditions, has drawn criticism from commentators and some lawmakers in Germany.

"The ECB has in the past always kept to its mandate and I assume it will do so in the future, too," Schaeuble countered, according to Bild am Sonntag. "One thing is clear: monetary policy must not serve to finance states. That line must not be crossed."

As for the purchases' unlimited nature, "it seems obvious to me that the ECB cannot name any upper limits without positively challenging speculators," he added.

Asked about the possibility that having to fulfill new conditions might make countries reluctant to seek help, ECB executive board member Benoit Coeure underlined in an interview with France Inter radio the bank's desire that "the conditions be in place for our policies to be efficient."

"That doesn't necessarily mean more austerity," Coeure said. "These countries have, as we know, already taken a lot of measures that go in the right direction, so there will not necessarily be additional demands."

World record set for highest surface area material

ScienceDaily (Sep. 7, 2012) ? Northwestern University researchers have broken a world record by creating two new synthetic materials with the greatest amount of surface areas reported to date.

Named NU-109 and NU-110, the materials belong to a class of crystalline nanostructure known as metal-organic frameworks (MOFs) that are promising vessels for natural gas storage for vehicles, catalysts, and other sustainable materials chemistry.

The materials' promise lies in their vast internal surface area. If the internal surface area of one NU-110 crystal the size of a grain of salt could be unfolded, the surface area would cover a desktop. Put another way, the internal surface area of one gram of NU-110 would cover one-and-a-half football fields.

A paper describing the findings, "Metal-organic Framework Materials with Ultrahigh Surface Areas: Is the Sky the Limit?" was published August 20 in the Journal of the American Chemical Society.

The research team, led by Omar Farha, research associate professor of chemistry in the Weinberg College of Arts and Sciences, has synthesized, characterized, and computationally simulated the behavior of the two MOFs that display the highest experimental Brunauer-Emmett-Teller surface areas of any porous material on record, 7,000 m2/g; that is, one kilogram of the material contains an internal surface area that could cover seven square kilometers. (Brunauer-Emmett-Teller, or BET, is an analysis technique for measuring the surface area of a material.)

The extremely high surface area, which is normally not accessible due to solvent molecules that stay trapped within the pores, was achieved using a carbon dioxide activation technique. As opposed to heating, which can remove the solvent but also damage the MOF material, the carbon dioxide-based technique removes the solvent gently and leaves the pores completely intact.

The development could rapidly lead to further advances. MOFs are composed of organic linkers held together by metal atoms, resulting in a molecular cage-like structure. The researchers believe they may be able to more than double the surface area of the materials by using less bulky linker units in the materials' design.

The research comes from the labs of Joseph T. Hupp, professor of chemistry in Weinberg, and Randall Q. Snurr, professor of chemical and biological engineering at the McCormick School of Engineering.

Other authors include SonBinh Nguyen, professor of chemistry in Weinberg; Ibrahim Eryazici, Nak Cheon Jeong, Brad G. Hauser, Amy A. Sarjeant, and Christopher E. Wilmer, all of Northwestern; and A. ?zg?r Yazayd?n of the University of Surrey in the United Kingdom.

The MOF-designing and -synthesizing technology is being commercialized by NuMat Technologies, a Northwestern startup that has won more than $1 million in business plan competitions since incorporating in February.

The World Health Organization (WHO) estimates that there are 500,000 new cases of cervical cancer and 250,000 deaths from the disease worldwide each year.

Sprenger said European countries might have to examine why their HPV vaccination rates are not higher, and change their campaigns accordingly. Parental permission is required to administer the vaccine to young girls.

Convincing Parents

The Robert Koch Institute, a German federal health agency, has recommended since 2007 that girls between the ages of 12 and 17 receive the vaccine. Although the public health insurance program covers the costs of the vaccine, only between 30 and 45 percent of girls receive it, the agency estimates.

That percentage is too low, said Yvonne Deler?, with the Department of Infectious Disease Epidemiology at the Robert Koch Institute. "If one wants to reduce the number of cases of cervical cancer and its early stages among the population, a vaccination rate of probably 80 percent would be needed," she said.

Two European countries are currently meeting that goal, according to the ECDC survey: Britain and Portugal. Many of the other countries have a vaccination rate hovering around 17 percent.

In Europe, the factors deterring people from choosing the vaccine are often its price tag and the fact that three separate shots have to be given over a period of six months, says the ECDC.

HPV is a group of viruses, at least 14 types of which are classified as "high risk" and can cause cervical cancer in women. According to the U.S. Centers for Disease Control and Prevention, it is a "common virus that is easily spread by skin-to-skin contact during sexual activity with another person."

Two types of HPV, types 16 and 18, cause about 70 percent of cervical cancers. The two vaccines licensed for use in Europe -- Cervarix offered by Britain's GlaxoSmithKline and Gardasil offered by Merck & Co. from the U.S. -- protect against both types. They do not, however, cure existing infections, so they should be given to girls before they become sexually active. Merck's vaccine also guards against other strains of the virus that cause genital warts.

Political Issue

In the U.S., the HPV vaccine has been a controversial political issue, with some public officials questioning its safety, and social conservatives questioning whether or not it would promote sexual promiscuity.

Last year, it became a hot-button issue during the Republican presidential primary campaign in the U.S.. Rick Perry, Republican governor of the state of Texas, came under fire from his opponents for having once issued a mandate requiring 6th grade girls in Texas to get the vaccine.

One of his opponents, Michele Bachmann, later had to backtrack after saying on television that the vaccine can have dangerous side effects and relaying a story about a woman who blamed it for her daughter's mental retardation.

The American Academy of Pediatrics blasted her comments as having no scientific basis, and has recommended the vaccines for both girls and boys between 11 and 12 years old.

The ECDC stopped short this week of recommending the vaccine for boys -- HPV can cause penile and anal cancers, among others -- because, it said, "including boys in the current HPV vaccination programs is unlikely to be cost-effective."
